I am classifying a failure as a request that has gone over 1 calendar year without a response.

Any home requests I sent that were different in the 04 Meiselman than the 03 Meiselman I have listed b\c possibly I just sent this stuff to homes where they don't live and the forwarding expired?

If it says "sent to home" then that means the addy I sent it to in 03 is the same addy in the 04 Harvey. 

If it says a city and state that means I sent it to the 03 addy and the 04 Harvey is different.
